What to include

Describe the problem, the questionnaire link, approximate time, browser and device. For access, correction or deletion, explain which record your request concerns. Share only the information needed to investigate. Never send a password, recovery link, full account export or sensitive answers in a support message.

Trouble submitting

If the page cannot confirm a submission, keep it open and use its retry action. Reloading can discard unsent answers. If a questionnaire is unavailable, check with the person who shared it. For suspected abuse or a security problem, stop using the affected questionnaire and contact the operator privately.
